Q: Who needs to complete the Lead Certificiate?

             A: EVERYONE!
            There is a place on the form to record
         *Dates of the blood testing
          *Exemption from the test. if you have never lived in an
            at-risk zip code.
 

Q: When are the tests done?

A: The documented tests should be at 12 months and 24 months of age

Q: My child had her first lead test at 13 months does she need a second test?

A: Yes. If the first test was done before 24 months of age, two test results are required. 
If the first test is done  after 24 months of age one test
is required.
